RUFSD NEW STUDENT REGISTRATION REQUIREMENTS 

The following original documents MUST be presented by the parent/guardian at the time of returning student registration. Note, it is your responsibility to obtain all original documents.  Forms Must Be Current – Originals Only. You Must Have Picture Identification. 

• PROOF OF BIRTH 
• PROOF OF GUARDIANSHIP:   

• _______DSS Form DSS2999   ______Agency Letter with CIN# and Case# 

• CURRENT HEALTH AND IMMUNIZATIONS FORMS SIGNED BY THE DOCTOR 
• LAST REPORT CARD(K-8TH); TRANSCRIPT (8TH-12TH); WITHDRAWAL/TRANSFER SLIP (If applicable)
• PROOF OF RESIDENCY: 

• _______ Mortgage Statement                  or       _______ School/Property Taxes          or            _______ Current Notarized Lease  and 

• Two (2) of the following items below 
• Four (4) of the following items below, if you are renting, but do not have a written rental or lease agreement or do not have any utility bills, you must provide four (4) of the following items listed below (not including: PSEG, KEYSPAN, Long Island Water, Nassau County Tax bill, Homeowner’s Insurance Policy). Your landlord will be required to complete a landlord affidavit form and submit required proofs of ownership as listed above.  All affidavits must be notarized. (* = Third or Fourth Option) 

Must Be Current – Originals Only Plus Photo Identification

Los siguientes documentos originales DEBEN ser presentados por el padre/encargado al momento de la inscripción del estudiante que regresa. Tenga en cuenta que es su responsabilidad obtener todos los documentos originales.  Los formularios Deben Ser Actuales – Originales Solamente. Usted Debe Tener Una Identificación con Foto. 

• PRUEBA DE NACIMIENTO 
• PRUEBA DE TUTELA (SI ES APLICABLE):  

• Formulario DSS2999 del Departamento de Servicio Social 

• Carta de Presentación de la Agencia con CIN# y Caso# 

• FORMULARIO ACTUAL DE SALUD E INMUNIZACIONES FIRMADO POR EL MÉDICO 
• ULTIMA TARJETA DE NOTAS (K- 7TH);TRANSCRIPCIÓN DE CRÉDITOS (8TH-12TH); DOCUMENTO DE TRASLADO 
• PRUEBA DE RESIDENCIA:  

• _______Documento de la Hipoteca _______Impuestos Escolares /de Propiedad   _______Contrato de Arrendamiento Actualizado y debidamente Notarizado  y 

• Dos (2) de los siguientes documentos o 
• Cuatro (4) de los siguientes documentos Si está alquilando, pero no tiene un contrato de alquiler o arrendamiento por escrito o no tiene facturas de servicios públicos, debe proporcionar cuatro (4) de los siguientes documentos enumerados a continuación (sin incluir: PSEG, National Grid, Long Island Water, Factura de Impuestos del Condado de Nassau, Póliza de Seguro del Propietario). Su arrendador deberá completar un formulario de declaración jurada de arrendador y presentar las pruebas de propiedad requeridas como se detalla anteriormente. Todas las declaraciones juradas deben ser notarizadas. (*= Tercera o cuarta opción)
